Incident Investigation Training

This Incident Reporting and Investigation Training is designed to give participants a greater understanding of the steps involved with both internal and external incident investigations. Participants will analyse real-life accident scenarios and practice Immediate Action Drills (IAD’s) on hypothetical accidents. It looks at both reactive and (more importantly) pro-active approaches to incident management.

This four hour introductory Incident Reporting and Investigation course can be delivered to your company group on-site or via Zoom.

Course Topics – Introduction to Incident Reporting and Investigation

  1. Types of Incidents – Defining incidents, why we need to investigate them and which are reportable (and more importantly how to report them) to the WHS Regulators.
  1. Incident Causation Theories – Why incidents occur, the six causational factors, swiss cheese model, tree diagrams and root cause analysis explained via simple, practical exercises.
  1. Preparing for an Incident Investigation – Steps to take before the investigation begins, including incident scene security, appointing the investigation team, resources and logistics.
  1. Undertaking an Investigation – Steps to take during the investigation, including witness identification, interview preparation, gathering facts, taking photographs, causation identification and analysis and preparing an investigation report.
  1. Learning from Incidents – Steps to take after the investigation, including implementing and communicating improvements, updating procedures, equipment and training.

Who should attend Incident Investigation training?

This course is designed for anyone responsible for investigating and/or analysing incidents or tasked with developing corrective action to prevent re-occurrences. As this responsibility normally falls on most line managers, this course is essential for anyone who supervises others.

Delivery methods: This training is available for company groups and can be delivered face-to-face at your company site or via Zoom.

On-site courses can be delivered in Sydney and across NSW. Our trainers can also travel to Melbourne, Brisbane, Canberra, Perth and Adelaide.
